In the next couple of months, I will be at the St. Joseph Community Health Foundation weekly for three hours. St. Joseph is an organization that is sponsored by the Poor Handmaids of Jesus Christ. They mainly serve those that are in impoverished in Allen County by answering the needs of the communities with grants, collaborate with other local organizations to help other underserved populations, and work on other projects that improve and help in maintaining the long-term health of the community.
- Kevin This semester I will be spending Thursday mornings at the Park Center located on State Blvd. here in Fort Wayne. After having my first visit to the site, I have learned that it is a facility that cares primarily for patients of all ages who have a variety of mental illnesses. Park Center has inpatient care, outpatient care, transitional care, and also ambulatory care. They also are able to provide housing for people who cannot afford it on their own. The clinic is currently expanding and incorporating integrated care into their site. They have recently hired a new physician, Dr. Williams, who is a primary care physician. The site hopes to have physicians of all practices on site in the near future. My journey throughout the semester will allow me to shadow once inpatient and once in an outpatient setting to get a feel for the majority of the patients that are seen at the clinic. Dr. Williams has previous experience at a similar clinic, Oak Lawn, and strongly believes in patient education. With her I will be working to develop a variety of patient education resources. This will likely be an ongoing project.
- Kadie My expectations for this course is to gain knowledge on vulnerable and underserved patient populations. I hope to be able to become somewhat familiar with each of these patient populations so I can better serve them in the future as a pharmacist. I would also like to improve my communication skills throughout the course of this semester as it will help me talk to future patients and develop rapport. When your patient trust you, they'll be more comfortable sharing their health history and other pertinent information so you can better help them.
